ASCEND BioCare Portal
Terms of Use
These Terms of Use apply to your use of the ASCEND BioCare Portal.
The ASCEND BioCare Support Program is sponsored by GlaxoSmithKline Australia Pty Ltd (GSK) and administered by Inservio Home Pty Ltd (Inservio Home).
The Portal allows:
- patients or their authorised representatives to submit a self-enrolment request; and
- eligible healthcare professionals to submit a patient referral.
The Portal also enables eligible patients and healthcare professionals to access services and support available through the ASCEND BioCare Support Program.
By using the Portal or submitting a form, you agree to these Terms of Use.
1. Use of the Portal
The Portal is primarily a form-submission service. It does not provide user accounts or ongoing access to information previously submitted through the Portal.
Submitting a form does not:
- automatically enrol a patient in the Program;
- confirm that a patient is eligible;
- guarantee access to any medicine or Program service;
- validate a prescription; or
- replace advice from a healthcare professional.
Once a form is submitted, Inservio Home will contact the patient, authorised representative or healthcare professional to confirm the information provided and request any additional details required to assess the enrolment request or referral.
2. Program services
Services provided through the ASCEND BioCare Support Program may vary depending on:
- the Program information provided;
- the patient's individual needs; and
- the GSK treatment prescribed to the patient.
Services provided through the Program may include:
- delivery of medication
- pharmacists counseling associated with a GSK product dispensing;
- S100 application support;
- patient access to sharps containers and materials associated with the use of a GSK product;
- patient education and self-injection training provided virtually by a nurse;
- prescription repeat reminder services;
- patient and HCP access to online information, tools and resources;
- patient and HCP access to customer support services provided by the Inservio Home team.
The services available to an individual patient will depend on their eligibility, treatment and support needs.
3. Patient self-enrolment
The patient self-enrolment form may be submitted by:
- the patient; or
- a person authorised to act on the patient's behalf.
By submitting the form, you confirm that:
- the information provided is accurate to the best of your knowledge;
- you are authorised to provide the information;
- you agree to be contacted by Inservio Home about the enrolment request; and
- you understand that submitting the form does not confirm enrolment in the Program.
Inservio Home may request further information or evidence confirming that an authorised representative is permitted to act on behalf of the patient.
4. Healthcare professional access and referrals
The healthcare professional section of the Portal may only be viewed, accessed and used by healthcare professionals who are registered to practice in Australia.
The patient referral form may only be submitted by a registered healthcare professional involved in the patient's care.
By submitting a referral, the healthcare professional confirms that:
- they are authorised to submit the referral;
- they are involved in the patient's care;
- the patient has been informed about the referral;
- the patient has agreed to their information being provided;
- the information submitted is accurate to the best of their knowledge; and
- they agree to be contacted by Inservio Home about the referral.
The referral form must not be submitted by clinic, hospital or administrative staff on behalf of the healthcare professional.
5. Information you provide
You must provide information that is accurate and complete to the best of your knowledge.
You must not:
- submit false or misleading information;
- impersonate another person or healthcare professional;
- submit another person's information without authority;
- use another healthcare professional's AHPRA number;
- submit a referral without the patient's knowledge or agreement; or
- misuse, disrupt or interfere with the Portal.
Incomplete or inaccurate information may delay or prevent an enrolment request or referral from being processed.
6. No medical advice
The Portal is provided for Program enrolment, referral and administration purposes only.
The Portal, GSK and Inservio Home do not provide medical advice through the form-submission process.
The Portal must not be used to:
- diagnose a medical condition;
- assess symptoms;
- determine whether a medicine is suitable;
- start, stop or change treatment; or
- obtain emergency medical assistance.
Patients should contact their treating healthcare professional with questions about their condition, medicine or treatment.
In a medical emergency, call Triple Zero on 000 or attend the nearest emergency department.
7. Opting out of the Program
A patient's participation in the ASCEND BioCare Support Program will continue for the duration of the Program unless the patient chooses to opt out or their participation otherwise ends.
Patients may opt out at any time by:
- calling 1300 803 901 and selecting Option [4]; or
- emailing the ASCEND BioCare Support Program at ascendbiocare@inserviohome.com.au.
Opting out may mean that some or all Program services will no longer be available.
GSK or Inservio Home may also suspend or end a person's participation in the Program where reasonably necessary, including where eligibility requirements are no longer met or for operational, regulatory or safety reasons.
Where practicable, reasonable notice will be provided.
8. Privacy
Personal information and health information submitted through the Portal will be collected, used and disclosed by Inservio Home and GSK for the purpose of administering the ASCEND BioCare Support Program.
When you provide personal information or health information through a form on the Portal, you should read the privacy collection notice included with that form. It explains what information is collected, why it is collected and how it may be used or disclosed.
By submitting a form, you acknowledge that you have read the privacy collection notice provided with that form.
If you provide information about another person, including a patient, you confirm that you are authorised to provide that information and have directed the person to the Privacy Collection Notice and the Inservio Home Privacy Policy.
9. Adverse event reporting
The ASCEND BioCare Portal is not intended or designed to record or report adverse events.
To report an adverse event related to a GSK product, please contact:
- GSK Australia: 1800 033 109; or
- Therapeutic Goods Administration: www.tga.gov.au/safety/report-problem/report-adverse-event-or-safety-problem.
In a medical emergency, call Triple Zero on 000 or attend the nearest emergency department.
10. Changes to the Program and these Terms
These Terms of Use may be updated from time to time, including where Program eligibility requirements, services or processes change.
The current version will be published on the Portal together with its effective date.
Where practicable, Inservio Home may also notify participants of material changes, including by email.
Continued participation in the Program or use of the Portal after updated Terms take effect will constitute acceptance of the updated Terms.
GSK or Inservio Home may also update, improve or modify the Portal, Program services or supporting systems from time to time.
The Program may be suspended or terminated at the direction of GSK, including for regulatory, safety, operational or other reasons considered necessary by GSK.
